You are the most important person in our office and we are dedicated to making sure you feel that way! In addition, we are dedicated to providing you with compassionate, high-quality medical care. Please do not hesitate to ask us any questions that you may have.
Office Hours: Monday through Friday, 8:00am – 5:00pm. To better serve you, we are open through the lunch hour.
Emergencies: For life-threatening situations, please call 911. If you have an urgent issues, please call our office for instructions. Our answering service is available after hours and can assist you.
Medication Refills: Please contact your pharmacy for refill requests – they will fax us the request and your refills will be processed within 72 hours. You may also request refills on the Patient Portal.
Test Results: Most test results cannot be given over the phone. Your provider will discuss any results with you during your follow-up visit. You may also view test results on the Patient Portal.
Please arrive 30 minutes early to complete any necessary paperwork. Please make sure to bring copies of your insurance cards, photo ID, a list of current medications, and your copayment or deductible.
As a courtesy to other patients, please call the office as soon as possible if you are unable to keep your appointment.
Copays, co-insurance, out-of-network balances, deductibles, and payments for non-covered services are due at the time of service. If you do not have insurance, payment is due at the time services are rendered. Payment plans can be arranged.
Prior to your appointment, please check your insurance coverage to be aware of any referral requirements, copays, deductibles, or other network requirements. If you have an HMO or an insurance plan that requires a PCP referral or authorization, please verify that the referral has been sent to our office prior to your appointment. Lack of referral may result in your appointment being canceled.
